Need to request service? Please download and fill out the Application for Service.  Just print and complete the form, and return it to Manitowoc Public Utilities.  Either mail the form, drop it off at our facility, email a scanned copy, or use our fax number below. As soon as we receive your information, a representative will be in touch to get you set up. A deposit may be required to start service.

New Construction?

For new construction, also download and fill out the service spot certificate of inspection or affidavit form.    Attach site plan drawings or a sketch of the property to be served.  Include a desired service location(s), if any, and a load profile if the building requires a service larger that 200 amps.
